Job summary

Brooks Rehabilitation in Jacksonville, FL seeks a Nurse Case Manager for the SCI/D Day Treatment Program. The role coordinates rehabilitation services, supports discharge planning and acts as a liaison between patient, the care team, payer, and community.

You will work in an interdisciplinary setting with an emphasis on community reintegration and optimizing outcomes for spinal cord injury patients; Florida RN or LPN license required with 2 years of case management experience.

Qualifications

• RN or LPN license required in Florida.
• Minimum 2 years of Nurse Case Manager experience in Acute Care or Inpatient Rehabilitation.
• Experience with spinal cord injuries is preferred but not required.
• Candidates must be flexible to meet patient and family needs and coordinate with multiple stakeholders.

Responsibilities

• The Nurse Case Manager coordinates rehabilitation services to meet individual needs.
• Promotes positive outcomes by linking patient, team, payer, and community.
• The plan-of-care facilitates communication and smooth transitions back to the community.
