Generations Healthcare is seeking a Nurse Case Manager (LVN/RN) with skilled nursing facility experience to join our Temecula team. The role involves developing, evaluating, and implementing patient care plans, coordinating with families, doctors, providers, and patients to ensure cost-effective medical plans and high-quality care.

Nurse Case Manager(LVN/RN)w/ Skilled Nursing Facility Experience

Wage: $45-55/hr DOE

aGenerations Healthcare facility is in search of aNurse Case Manager to join our team.

The primary purpose of theNurse Case Manager is to develop, evaluate, implement health care plans for individual patients, manage cost effective medical plans, while providing the highest level of patient care. The Case Manager also communicates treatment plans between families, doctors, providers and patients.

Essential Duties (include but are not limited to):

• Evaluate and update patient health care plans
• Assessment for new patients
• Review test results from medical exams
• Implement Physician orders and follow up with treatment plans
• Communicate with families, health providers and patients
• Documentation of medical records
• Provide emotional support to families and patients
• Provide education about health care plans to patients and families

Qualifications:

• Must possess, as a minimum, a Nursing Degree from an accredited college or university.
• Must have, as a minimum, one (1) years of experience as a supervisor in a hospital, nursing carefacility, or other related health care facility.
• Must have training in rehabilitative and restorative nursing practices.
• Strong communication, excellent customer service, teamwork and organizational skills
• Must possess a current, unencumbered, active license to practice as an RN in this state.
• Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
• Be a supportive team member, contribute to and be an example of team work and team concept.
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
• Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
