Heritage Park Nursing Center in Upland, CA is seeking a Licensed Vocational Nurse (LVN) for the PM shift (3pm-11pm) with weekend availability and behavioral health experience. The role focuses on delivering skin care under medical supervision and coordinating care with the nursing leadership.

Qualified candidates hold a California LVN license, with prior SNF or behavioral health experience preferred. Excellent teamwork and patient-centered communication are essential for success.

Rate: $28-$35/HR DOE

Heritage Park Nursing Center, a Generations Healthcare facility, is in search of a Licensed Vocational Nurse to join our team with behavioral health experience.

Job Description:

The primary role of the Licensed Vocational Nurse is to provide primary skin care to residents under the medical direction and supervision of the residents’ attending physicians, the Director of Nursing Services, or the Medical Director of the facility, with an emphasis on treatment and therapy of skin disorders. You will also assist in modifying the treatment regimen to meet the physical and psycho-social needs of the resident, in accordance with established medical practices and the requirements of this state and the policies and goals of this facility.

Qualifications:

• Current active license to practice as a Licensed Vocational Nurse in the state of California.
• Experience in behavioral health preferred.
• Prior Skilled Nursing Facility experience or Behavioral Health.

• Strong communication, teamwork and organizational skills.
