Qualifications

Responsibilities

• Perform utilization review and discharge planning for inpatients.
• Apply Interqual, AEP, and other criteria to determine appropriate care.
• Collaborate with care teams and community case managers.
• Coordinate post-acute care planning and patient transitions.
• Participate in quality improvement and regulatory activities.
