Qualifications

Responsibilities

• Perform utilization review and discharge planning for inpatient admissions.
• Apply Interqual, AEP and clinic criteria to determine appropriate care level.
• Determine appropriate level of care and lead the healthcare team in care options.
• Collaborate with Community Case Managers on medically fragile/high-risk patients.
• Identify delay days and report to Team Leader; support post-acute planning.
• Coordinate with physicians and post-acute care to ensure timely access.
• Participate in committees and quality improvement activities.
