Job summary
Beth Israel Lahey Health seeks a skilled Registered Nurse for community/home health in Massachusetts. You will plan, organize, and direct home care services, applying clinical expertise and community resources to meet patient and family needs.
The role emphasizes initiating and adapting plans of care, monitoring patient progress, administering treatments, and ensuring proper documentation in EPIC. A BLS credential and an active MA RN license are required.
Qualifications
- Active Massachusetts RN license required.
- Active American Heart Association BLS certification.
- Associate’s Degree or BSN from accredited nursing program.
- At least 1 year of direct care nursing experience, home care preferred.
Responsibilities
- Complete initial patient/family assessment to determine home care needs.
- Initiate and revise the plan of care as patient status changes.
- Administer medications and treatments as prescribed.
- Instruct, supervise and evaluate home health aide care every two weeks.
- Identify discharge planning needs as part of care planning.
- Maintain confidentiality per agency policy and HIPAA guidelines.
- Demonstrate proficiency in OASIS completion.
- Document in EPIC per agency timeliness policies.
