Job summary

Medical University of South Carolina seeks a Nurse Case Manager I (PRN) to coordinate an interdisciplinary care plan from admission through discharge in a hospital setting. Job Description Summary

The Nurse Case Manager (NCM) I reports to the Manager of Case Management and Care Transitions. Under guidance of the Nurse Case Manager Team Leader, the Nurse Case Manager I functions as a member of the clinical team. This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge. This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system. The Nurse Case Manager I serves as a clinical expert related to discharge planning, resource utilization, and case management.

Job Summary/Purpose

Coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews.


Minimum Training and Education

Bachelor's degree in Nursing from an accredited school of nursing and three years work experience as a Registered Nurse required. Prior leadership experience preferred. One year of case management and/or utilization review work experience preferred. Case Management certification preferred.


Required Licensure, Certifications, Registrations

Licensure as a registered nurse by the state of South Carolina or compact state required. Current Basic Life Support (BLS) or ACLS required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.
