Responsibilities

• Provides case management services to a caseload of low- and medium-risk patients with interdisciplinary collaboration.
• Interviews patients and caregivers to evaluate needs, goals, and current services; develops care plans.
• Assists patients with gaining access to care, makes referrals, and coordinates resources for continuity and quality of care.
• Updates authorizations and diagnoses; attends case management rounds with clinicians; monitors progress toward goals.
• Verifies services comply with guidelines and documents cases in medical files.
• Identifies barriers to goals and engages patient and care team to address them.
