Job summary

Qualifications

• Preferred qualification: Registered Nurse license.

Responsibilities

• Provides case management services to a limited caseload of low-risk patients.
• Coordinates with patients, caregivers, and healthcare team to evaluate needs, goals, and services.
• Determines initial eligibility, benefits, and education for admissions and assesses medical necessity for hospital admission.
• Documents the patient’s case in all medical files and ensures services comply with guidelines and standards.
• Assists with disposition planning and develops comprehensive plans in collaboration with patients, families, and providers.
