Partnership HealthPlan of California is seeking a registered nurse to lead case management, coordinating multidisciplinary care plans for members while addressing clinical and psychosocial needs. You will collaborate with providers and care teams to ensure timely referrals, documentation, and resources.

Ideal candidates hold an active California RN license, prefer BSN, and have prior case management and acute care experience in managed care settings.

Qualifications

• Associate's degree in Nursing required; BSN preferred.
• Current California RN license required.
• 2 years of case management experience and 1 year in acute care or equivalent.

Responsabilités

• Provide independent case management for a caseload ranging in complexity.
• Initiate and coordinate individualized care plans for members.
• Communicate with providers and members to ensure continuity of care.
• Coordinate referrals and authorizations for services.
• Maintain documentation in the Case Management System.
• Adhere to NCQA documentation standards.
