Job summary

Partnership HealthPlan of California seeks an experienced Registered Nurse Case Manager to coordinate a multidisciplinary team approach to case management and to engage members and their representatives in effective care planning.

You will assess, plan, facilitate, and advocate for comprehensive medical, behavioral, and psychosocial needs to achieve quality, cost-effective outcomes. CA RN license and 2 years of case management experience required; travel may be needed.

Qualifications

• Associate degree in Nursing required.
• CA RN license required.
• 2 years of case management experience preferred.
• Willingness to travel as needed.

Responsibilities

• Provides independent case management for a caseload with varying complexity.
• Initiates and coordinates individualized care plans for assigned members.
• Resolves member needs using multidisciplinary team strategies.
• Maintains documentation and coordinates referrals and authorizations.
• Collaborates with providers, members, and internal departments to ensure continuity of care.
