Job summary

The Medical University of South Carolina is seeking a Nurse Case Manager II to join the Case Management & Care Transitions team. This role coordinates a comprehensive plan of care, supports safe and timely discharges, and acts as a clinical expert in discharge planning and resource utilization within the hospital system.

You will report to the Manager of Case Management and Care Transitions, work a 40-hour week, Monday–Friday with occasional weekends or holidays.

Qualifications

• Bachelor’s degree in Nursing from an accredited school and three years of RN work experience.
• Case Management experience preferred. Leadership experience preferred.
• Case Management certification preferred.

Responsibilities

• Coordinate a plan of care to achieve clinical outcomes and promote safe, timely discharge.
• Serve as a clinical expert in discharge planning, resource utilization, and case management.
• Ensure efficient movement of the patient through the hospital system.
