UnitedHealth Group is seeking a Nurse Case Manager in Chesapeake, VA to guide patients and families through the care continuum, assess needs, coordinate care, support discharge planning, and manage ongoing outpatient care under supervision of an RN or MD.

You will help ensure appropriate level of care, provide education, coaching, and decision support, and collaborate with a Care Advocate and Social Worker. This role offers a comprehensive benefits package and growth opportunities.

Qualifications

• Current RN license, state(s) you are assigned to work
• 1+ year of clinical experience as a Registered Nurse (RN)
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance

Responsabilités

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other disciplines as indicated by the patient's identified needs
• Promotes patient health and independence through teaching and rehabilitation measures
• Supervises LPNs and HHAs
• Completes documentation timely and accurately
• Regularly communicates patient progress to the clinical manager and care team
• Plans and provides staff education
• Performs on-call responsibilities and on-call services to patients/families
• Participates in clinical development and continuing education programs
• Other duties as assigned
