HCA HealthONE Sky Ridge in Colorado is seeking a Case Manager (RN) to support patients and families through care transitions. You will coordinate services, develop care plans, and collaborate with physicians, nurses, and care teams to ensure safe, patient-centered care.

Responsibilities include guiding patients through orientation, educating families, creating individualized plans, completing psychosocial assessments, and communicating needs across the team.

Qualifications

• Licensed RN in current state – required.
• Case Management certification preferred.
• Associate Degree in Nursing or nursing diploma – required.
• BSN preferred.
• 2+ years in case management or 3+ years in clinical nursing – required.
• InterQual experience preferred.

Responsibilities

• Guide patients and families through orientation, benefits, discharge criteria, and rights.
• Coordinate education for patients and caregivers to support care planning.
• Develop and implement individualized treatment plans reflecting needs and goals.
• Complete psychosocial assessments and discharge planning; document findings and share with team.
• Collaborate with Rehab Program Director and care team for seamless care.
• Promote compassion, respect, and excellence in all patient interactions.
