Job summary

Qualifications

• Two years of full-time direct care experience and/or hospital-based case management experience.
• Registered Nurse license in South Carolina; BLS certification.
• Proficiency with Word, Excel, PowerPoint and Access; strong planning & organizational skills; excellent written and verbal communication; ability to work independently.

Responsibilities

• Review patient history and current status to determine care needs.
• Conduct case screening within 24 hours of admission.
• Interview patients and families to identify health and psychosocial needs.
• Evaluate treatment plans and post-acute care options.
• Identify barriers to wellness and resources, including insurance benefits.
• Develop individualized care management plans with patient/family input.
• Assess age-specific and special needs; ensure appropriate teaching for patient goals.
