Qualifications

• Bachelor's degree and one year work experience; or high school diploma (or equivalent) and 2 to 4 years work experience.

Responsibilities

• Supports RN Case Managers and Social Workers in their work with the interdisciplinary healthcare team across the continuum.
• Coordinates payer communication, transition plan implementation, and delivery of patient documentation.
• Performs other duties in support of case management departmental functions.
