Job summary

HomeCentris Healthcare, LLC is seeking a PRN RN Case Manager in Owings Mills, MD to provide nursing oversight for nonskilled homecare services and coordinate patient care in clients’ homes.

The RN uses assessment skills to plan and document care for an assigned caseload and develops home health plans that meet patient needs in compliance with regulations.

Qualifications

• Registered nurse with active licensure in MD (as implied in role).
• Certified Delegating Nurse is preferred.
• 1 year of case management in home health.

Responsibilities

• Coordinate care to assigned clients, establish Start of Care plan, and perform ongoing assessments.
• Conduct onsite supervisory visits to assess client, environment, and caregivers, with timely documentation.
• Provide guidance, supervision, training, education, and evaluation of caregivers; deliver excellent customer service.
• Maintain a professional demeanor consistent with RN standards of practice.
• Perform other duties as assigned by supervisor.

HomeCentris Healthcare is seeking a PRN RN Case Manager for our Personal Care division in our Lanham office to provide quality care and nursing oversight of its nonskilled homecare services in the setting where our clients most want to be, in their homes. The RN uses assessment skills to plan, coordinate, provide and document care for an assigned caseload of patients. The RN participates in the development and implementation of a home health plan that meets each patients' specific needs and is in compliance with federal and state regulations and agency policies and procedures.

RESPONSIBILITIES

• Coordinate care to assigned clients, establish a goal-directed care plan/Start of Care, and ongoing assessments of clients' needs.
• Perform onsite supervisory visits to assess client, family, environment, and clinical care givers and complete timely follow-up documentation.
• Provide guidance, supervision, orientation, training, education, and evaluation of caregivers. Provide excellent customer service.
• Maintain a professional demeanor consistent with registered nurse standards of practice.
• Perform other duties as assigned by supervisor.
