Job summary

Swift Staffing is seeking a Registered Nurse Case Manager for a government client in Towson, MD. This contract position covers case management for up to 50 clients awaiting appointments, with a duration of six to twelve months and potential extension or contract to hire.

The role pays $37.00 per hour with weekly pay, on-site schedule 8:30 am–4:30 pm, and a benefits package including health insurance, bonuses, and a Roth 401K plan.

Qualifications

• Bachelor's degree in Nursing required.
• At least one year of case management or nursing experience required.
• Maryland RN license with active BLS certification required.
• Must pass FBI background check and drug test.

Responsibilities

• Provides nursing assessments, person-centered care planning and referrals.
• Reviews LTSS assessment requests upon case assignment.
• Contacts assigned clients/representatives in a timely manner.
• Completes nursing assessments and care planning for cancer patients.
• Monitors and evaluates ongoing care plans for clients.
• Assesses health status and coordinates community resources.
• Ensures compliance with state, federal and Medicaid regulations.
• Provides health education and counseling on disease prevention and mental health.

Job description

Registered Nurse Case Manager needed for our government client in Towson. The Registered Nurse Case Manager will provide case management for up to 50 clients who are pending appointments. Six to twelve month contract position with possible extension or contract to hire consideration. The non-negotiable pay rate is $37.00 per hour.

Essential Duties:

• Provides nursing assessments, person-centeredcare planning and referral services to individuals, families, groups,and institutions.
• Reviews Maryland Department of Health LongTerm Services and Supports Tracking System (LTSS) assessment requestsupon assignment of cases.
• Contacts assigned clients/representatives timely.
• Completesnursing assessments and high-quality person-centered care planningtimely for those going through cancer treatments.
• Provides case management, ongoing monitoring and evaluation of person-centered care plan.
• Assesses and monitors clients' physical and mental states of health and determines need for community-based resources.
• Ensures compliance with state, federal and Medicaid regulations.
• Provides health screening education and counseling in the areas of disease prevention, health maintenance, and mental health.
• Prepares patient reports.

Qualifications:

• Graduation from an accredited college or university with a Bachelor's Degree in Nursing is required.
• At least one year of case management or nursing experience is required.
• Possession of a valid license to practice as a Registered Nurse by the Maryland State Board of Examiners of Nurse along with Basic Life Support certifications are required.
• Must be able to work 8:30 am-4:30 pm Monday through Friday with a 30-60 minute non paid lunch on site.
• Must be able to pass an FBI background check and drug test.
