Job summary

Swift Staffing seeks a Registered Nurse Case Manager for a government client in Towson. This contract position supports up to 50 clients awaiting appointments, with a six to twelve month term and potential extension or contract-to-hire.

The role offers a non-negotiable pay rate of $37.00 per hour, weekly pay, health insurance, referral bonuses, a longevity bonus, holiday pay and a Roth 401K plan. On-site schedule is Monday through Friday, 8:30 am to 4:30 pm.

Qualifications

• Bachelor's Degree in Nursing from an accredited program.
• Minimum 1 year of case management or nursing experience.
• Valid Maryland RN license with BLS certification.
• Availability 8:30 am–4:30 pm, Monday–Friday, on-site.
• Must pass FBI background check and drug test.

Responsibilities

• Provides nursing assessments, person-centered care planning and referral services.
• Reviews LTSS assessment requests upon assignment of cases.
• Contacts assigned clients/representatives timely.
• Completes nursing assessments and high-quality person-centered care planning, including cancer treatments.
• Provides case management, ongoing monitoring and evaluation of care plans.
• Assesses and monitors clients' health status and community resource needs.
• Ensures compliance with state, federal and Medicaid regulations.
• Provides health screening education on disease prevention, health maintenance, and mental health.
• Prepares patient reports.

Registered Nurse Case Manager needed for our government client in Towson. The Registered Nurse Case Manager will provide case management for up to 50 clients who are pending appointments. Six to twelve month contract position with possible extension or contract to hire consideration. The non-negotiable pay rate is $37.00 per hour.

Essential Duties:

• Provides nursing assessments, person-centeredcare planning and referral services to individuals, families, groups,and institutions.
• Reviews Maryland Department of Health LongTerm Services and Supports Tracking System (LTSS) assessment requestsupon assignment of cases.
• Contacts assigned clients/representatives timely.
• Completesnursing assessments and high-quality person-centered care planningtimely for those going through cancer treatments.
• Provides case management, ongoing monitoring and evaluation of person-centered care plan.
• Assesses and monitors clients' physical and mental states of health and determines need for community-based resources.
• Ensures compliance with state, federal and Medicaid regulations.
• Provides health screening education and counseling in the areas of disease prevention, health maintenance, and mental health.
• Prepares patient reports.

Qualifications:

• Graduation from an accredited college or university with a Bachelor's Degree in Nursing is required.
• At least one year of case management or nursing experience is required.
• Possession of a valid license to practice as a Registered Nurse by the Maryland State Board of Examiners of Nurse along with Basic Life Support certifications are required.
• Must be able to work 8:30 am-4:30 pm Monday through Friday with a 30-60 minute non paid lunch on site.
• Must be able to pass an FBI background check and drug test.
