Job summary

Alternative Wellness Services, Inc. is seeking an Office-Based BHH Nurse Care Manager (RN or LPN) in Maine. Part-time or full-time roles offer predictable weekday hours and no nights or weekends, enabling balance while coordinating care for clients with complex needs.

You will work with a multidisciplinary team to advocate for clients, manage health outcomes, and support safe transitions from hospital settings.

Qualifications

• Active RN or LPN license in Maine (or multi-state compact).
• Current CPR/BLS certification.
• Valid driver's license with reliable transportation.
• Experience in behavioral health or care coordination preferred.
• Ability to work autonomously within a multidisciplinary team.

Responsibilities

• Coordinate medical advocacy and care using the EHR; translate data into plans.
• Attend local doctor appointments to provide advocacy for clients.
• Provide health-literacy training to non-medical staff.
• Guide safe hospital/ER discharges and complete med reconciliations.
• Offer clinical guidance during crises and advise on protocols.
