Alternative Wellness Services in Maine is seeking an Office-Based BHH Nurse Care Manager (RN or LPN) for part-time or full-time roles. Enjoy predictable daytime hours, no nights, weekends, holidays, or on-call demands, and a collaborative multidisciplinary team.

Competenze

• Active Maine RN or LPN license required.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Proficient with EHR and standard office software.
• Excellent communication and ability to work autonomously in a team.
• Background checks completed; compliant with OIG.

Mansioni

• Coordinate medical care and health outcomes with a multidisciplinary team.
• Provide health-literacy training and support care plans.
• Facilitate safe discharges from hospitals or ERs and manage medication reconciliations.
