Unicity Care Management seeks a Nurse Care Manager to provide high-touch, in-home care coordination for clients on a private-pay caseload.

You will conduct comprehensive assessments, develop care plans, and coordinate with physicians, home care, and community resources while traveling to visits. This role emphasizes clinical judgment without in-office shifts and offers flexibility in building your week.

Competenze

• RN license valid in New Jersey.
• Strong clinical judgment for community settings.
• Excellent written and verbal communication.
• Excellent organizational skills to manage a caseload.

Mansioni

• Conduct comprehensive in-home assessments across clinical, functional and psychosocial needs.
• Develop and maintain individualized care plans and revise as needed.
• Coordinate physicians, home care, specialists and community resources.
• Manage transitions of care between hospital, rehab and home.
• Advocate for clients at appointments and family meetings.
• Serve as primary contact for your caseload; respond to urgent needs outside hours.
