Job summary

Qualifications

• Associates degree in Nursing is required.
• Two years of experience as a registered nurse are required.
• Clinical experience in geriatrics or managed long-term care preferred.

Responsibilities

• Assess and monitor members’ needs and status across the care continuum.
• Authorize services and coordinate care irrespective of payer.
• Collaborate with members, families, caregivers, and the IDT.
• Perform home visits to assess living conditions and needs.
