Job summary

CareSource is seeking a Nurse Care Manager in New York to coordinate care, conduct home assessments, and authorize services to ensure quality, cost-effective outcomes. You will work with members, families, PCPs, and the IDT to develop patient-centered plans and ensure appropriate discharge from the plan.

The role requires NY RN licensure, 2+ years of nursing experience, and potential bilingual capabilities. Frequent in-home visits and cross-venue care management are expected.

Qualifications

• Associates degree in Nursing required
• Bachelor's degree in Nursing preferred
• Two years RN experience required
• Geriatrics/long-term care experience preferred
• Bilingual ability may be required depending on operations

Responsibilities

• Assess and monitor members’ needs and status across the care continuum
• Authorize covered services and coordinate care regardless of payer
• Collaborate with member, family, caregivers, and IDT
• Perform home visits to assess living conditions and needs
• Develop patient-centered service plans with PCP and IDT
• Document care management per policy including recertifications
• Ensure safe discharge planning from the plan
• Identify and address complex care management needs in ICM
• Respond to member requests within designated timeframes
• Travel to member homes and various care settings as required
