Job summary

Suvida Healthcare seeks a Nurse Care Manager to join our multidisciplinary team. You will develop and implement care pathways for high‑risk patients, coordinate transitions of care, and oversee complex cases within assigned neighborhoods.

You will collaborate with physicians, nurses, and care teams to optimize outcomes and resource use. You will coordinate with external providers, review diagnoses, and ensure timely documentation and reporting.

Qualifications

• Bachelor’s degree in Nursing is required.
• Active RN license in Texas or a multi-state compact.
• Bilingual English/Spanish is preferred.
• Minimum 2 years of RN experience in utilization management, case management, discharge planning, transitions of care, or managed care.
• Strong knowledge of chronic care management and care coordination.

Responsibilities

• Oversees chronic care and transitions of care for high‑risk patients within care teams.
• Serves as a resource to the multidisciplinary team for complex patient management.
• Triage for patients via phone and route to appropriate interventions.
• Ensure efficient transitions from acute/post‑acute care to home or other settings.
• Perform comprehensive assessments of physical, mental, and social risk factors.
• Coordinate with providers and care team to remove barriers to care.
• Monitor discharge plans and communicate care progress to teams.
