Job summary

VNACJ CHC is seeking a Care Coordination Nurse Manager to lead our care coordination and case management programs. You will oversee the PCC team, optimize workflows, and drive population-health improvements within our outpatient setting in New Jersey.

You will collaborate with the Medical Director and Clinical Support teams, manage data-driven reporting, and ensure high-quality, patient-centered care. This leadership role emphasizes teamwork, compliance, and continuous improvement.

Qualifications

• RN license required; BSN preferred.
• Experience in population health or care coordination.
• 3–5+ years of clinical nursing experience (ambulatory or community health preferred).
• Strong Excel and data analysis proficiency.

Responsibilities

• Provide daily leadership for the Patient Care Coordinator (PCC) team.
• Ensure adequate coverage, efficient workflows, and adherence to organizational policies.
• Refine clinical and EHR workflows to improve patient outcomes.
• Track performance metrics and report to senior leadership.
