Job summary

Qualifications
- RN or LPN with chronic disease management experience
- Experience educating patients and supporting care coordination
- Licensure as an SC nurse (RN or LPN) required
Responsibilities
- Conduct face-to-face patient encounters and document in the EHR
- Assess barriers to accessing healthcare and provide patient education
- Coordinate care and communicate with interdisciplinary team
- Monitor patients after new medications or hospitalizations
- Support disease management and lifestyle goal setting with patients
