Job summary

Human Coalition in Raleigh, NC is seeking a Nurse Care Coordinator (LPN/RN) to provide in-clinic and telecare services for clients facing unplanned pregnancies. The role emphasizes compassionate, patient-centered care, education, resource connection, and follow-up within a multidisciplinary team.

The position requires a current nursing license (LPN or RN), CPR, and 2–4 years of clinical experience, with obstetrical/gynecological focus preferred.

Qualifications

• Must hold a current LPN or RN nursing license and be in good standing with the state medical board for the state in which you are applying for or a compact license equivalent.
• 2-4 years of prior clinical experience (Preferred).
• Emphasis on obstetrical and gynecological care preferred.
• Must have current and up to date CPR certification healthcare providers.

Responsibilities

• Effectively evaluate a client’s situation and circumstances over the phone to identify and address client chief complaint or concern.
• Provides education on abortion, parenting, and adoption services to patients over the phone and in person.
• Connect women with necessary resources to relieve pressure from circumstances.
• Utilizes knowledge base and compassion to provide quality follow up care to client.
• Perform limited medical services to women facing unplanned pregnancies.
• A key member of the medical team onsite providing quality, confidential and compassionate care for pregnant mothers.
• Offers counseling, education, and emotional support to clients and their family/significant other.
• Documents all client interactions and executes care plan accurately and timely in our electronic Client Management System (CMS).
• Communicates pertinent information to the Medical Director according to current organizational policies and procedures.
• Intervenes to promote client safety and security in manners consistent with the ethical and legal parameters of the scope of the professional nurse’s practice.
• Demonstrates patient centered/patient valued behaviors and must be capable of handling both medical and non-medical based questions utilizing independent judgment.
• Perform other duties as assigned
