Job summary

Human Coalition, Inc. in Raleigh, NC is seeking an LPN or RN Nurse Care Coordinator to join a weekday, onsite clinic team. You will provide non-diagnostic telephone and in-person assessments, education, and support to clients facing unplanned pregnancies, helping women and families access resources.

You will work under the Practice Manager, document care plans in the CMS, and cooperate with a compassionate medical team to ensure patient safety, confidentiality, and patient-centered care.

Qualifications

• Must hold a current LPN or RN license and be in good standing with the state medical board.
• 2–4 years of prior clinical experience; OB/GYN care preferred.
• Current CPR certification for healthcare providers.
• Excellent interpersonal and written/oral communication skills.
• HIPAA compliance knowledge.
• Ability to work in a fast-paced onsite weekday environment.

Responsibilities

• Assess clients by phone and in person to address chief concerns.
• Educate on abortion, parenting, and adoption services.
• Connect clients with resources to relieve pressure from circumstances.
• Document interactions and care plans in the CMS.
• Provide counseling, education, and emotional support to clients and families.
• Perform limited medical services and coordinate with the medical team.
• Communicate with the Medical Director per policies.
• Ensure patient safety and confidentiality.
• Other duties as assigned.
