Human Coalition in Grapevine, TX is seeking a Nurse Care Coordinator (RN/LVN) for a full-time onsite role. You will provide face-to-face, clinic and telecare services, performing non-diagnostic assessments, education, and support for clients facing unplanned pregnancies.

Responsibilities include evaluating client situations, delivering education on abortion, parenting and adoption, connecting women with resources, and documenting care plans in our CMS.

Competenze

• Must hold a current compact LVN or RN nursing license and be in good standing.
• 2–4 years of prior clinical experience in any specialty (preferred).
• Emphasis on obstetrical and gynecological care is preferred.
• Must have current and up to date CPR certification healthcare providers.

Mansioni

• Evaluate a client’s situation and circumstances over the phone to identify concerns.
• Provide education on abortion, parenting, and adoption services to patients over the phone and in person.
• Connect women with necessary resources to relieve pressure from circumstances.
• Utilize knowledge base and compassion to provide quality follow up care to client.
• Perform limited medical services to women facing unplanned pregnancies.
• Be a key member of the medical team onsite providing quality, confidential care for pregnant mothers.
• Offer counseling, education, and emotional support to clients and their families.
• Document all client interactions and execute care plans in CMS.
