Job summary

Ahwatukee Post Acute seeks an experienced nurse or social worker to support resident care planning and coordination. You will communicate with residents and families, participate in daily team huddles, and monitor patient status to adjust plans as needs change.

The role requires a Bachelor’s in Nursing or Social Work, with RN preferred and LVN/LPN acceptable. Strong documentation, compliance, and interprofessional collaboration are essential assets.

Qualifications

• Bachelor’s degree in Nursing or Social Work.
• RN license preferred; LVN/LPN license acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid programs and benefits.
• PCC knowledge.

Responsibilities

• Communicate with residents and families about plans of care, treatments, and status.
• Attend morning stand-ups to facilitate team communications.
• Monitor and adjust residents’ statuses based on changing needs.
• Organize daily work and prioritize resident care needs.
• Complete documentation accurately and timely.
• Coordinate admissions, discharges and referrals.
• Collaborate with care providers, financial services and third-party payers.
• Ensure compliance with policies and quality standards.
