Qualifications

• Valid New York State license and current registration to practice as a Registered Professional Nurse (NYSED).
• Baccalaureate degree in Nursing or related health field from an accredited college or university.
• Current Basic Life Support (BLS) certification from the American Heart Association (AHA).
• Two (2) years of experience as a Registered Professional Nurse.

Responsibilities

• Coordinates and monitors management of patient-centered quality care; facilitates progress from admission through discharge.
• Collaborates with physicians and health care professionals to optimize pathways of care and problem-solving.
• Communicates with reimbursement departments and payers to obtain authorizations and ensure appropriate reimbursement.
• Plans and implements strategies to reduce length of stay and resource use; coordinates discharge planning and post-discharge care.
