Job summary

Evergreen Retirement Community is seeking a compassionate Long Term Care Registered Nurse or Licensed Practical Nurse to join our team in Oshkosh, Wisconsin. The role involves delivering high-quality care for residents and collaborating with an interdisciplinary team on care plans.

Shifts are AM, two weeks per pay period, with Saturday and Sunday rotation. Qualified candidates hold an active Wisconsin RN or LPN license, with experience in long-term care preferred.

Job Overview

As a Long Term Care Nurse you will play a vital role in delivering high quality patient care and promoting the well being of our residents We are looking for a compassionate individual who thrives in a collaborative environment and is committed to making a positive impact in the lives of our residents This position is 2 shifts in a two week pay period on the AM shift 630am 245pm every other Saturday and Sunday

Key Responsibilities

• Provide holistic nursing care to residents in a long term care setting
• Assess plan implement and evaluate individualized care plans
• Administer medications and treatments as prescribed
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Foster strong relationships with residents and their families providing emotional support and education
• Maintain accurate medical records and documentation
• Participate in staff training and development activities

Qualifications

• Valid Registered Nurse RN or License Practical Nurse LPN license in Wisconsin
• Previous experience in long term care or similar healthcare setting preferred
• Strong communication and interpersonal skills
• Ability to work effectively in a team and independently
• Compassionate demeanor with a commitment to patient centered care
