Job summary

Evergreen At Home - Home Health is seeking a Registered Nurse or Licensed Practical Nurse to join our team. The RN/LPN will work in collaboration with a team of clinicians to provide skilled nursing services to patients in their own homes and will communicate with the patient's physician for critical updates.

The position offers 30-40 hours per week during the day with benefits including health, dental, and vision insurances, vacation, and 403(b) retirement plan matching.

Qualifications

• Valid Wisconsin RN or LPN license.
• Current CPR certification.
• Valid Wisconsin driver license.
• Reliable vehicle and insurance.
• Acceptable motor vehicle record.
• Experience with chronic conditions is a plus.
• Passion for working with older adults.
• Strong prioritization and organization skills.

Responsibilities

• Coordinate services and care for assigned clients in home health.
• Observe, report, and document signs, symptoms, and changes in patient condition.
• Educate clients and family on proper care and management of conditions.
• Arrange counseling services for client and family as appropriate.
• Complete OASIS documentation accurately.
• Assist in discharge planning and prepare discharge summaries.

Job description

Evergreen At Home - Home Health is looking for a Registered Nurse or License Practical Nurse to join our team! The RN/LPN works in collaboration with a team of clinicians to provide skilled nursing services to patients in their own homes and communicates regularly with the patient's physician to provide critical updates about changes in the patient's medical condition.

The available position is 30-40 hours per week during the day. It is also eligible for benefits, including health, dental, and vision insurances, vacation, 403(b) retirement plan matching, and much more!

Pay is hourly and not per visit. The wage range for LPN is $25.75-$32/hr. and RN is $37.50-42.50/hr.

Responsibilities Include

• Coordination of services and care for assigned clients (including therapy)


• Observation, recording, and reporting signs and symptoms, reactions to treatments and medications, and any change in the client's physical or emotional condition in a timely manner


• Teaching the client and family members about proper care and effective management of medical conditions


• Arrangement of counseling services for the client and family as appropriate


• Accurate completion of OASIS documentation


• Discharge planning and preparation of discharge summaries and instructions

Minimum Requirements

• Valid State of Wisconsin Registered Nurse or Licensed Practical Nurse license


• Current CPR certification


• Valid State of Wisconsin driver license


• Reliable vehicle and appropriate insurance


• Acceptable motor vehicle record


• Previous experience managing chronic conditions a plus


• Passion for working with older adults


• Strong prioritization and organization skills
