Cedar Ridge Village, a senior living community in West Des Moines, IA, seeks an Admissions Coordinator to assess referrals, plan admissions and advocate for resident-centered care. The role requires pre‑admission assessments, discharge planning, and collaboration with families, physicians and staff to uphold quality of life.

The position supports nursing leadership, participates in on‑call rotation, and helps coordinate care plans and allied health services for residents.

Qualifications

• Must be a graduate from an accredited school and licensed RN or LPN in the applicable state.
• Ability to communicate effectively with residents, families, physicians and staff.
• Strong organizational and multitasking abilities.

Responsibilities

• Follows the community’s philosophy and goal to maintain residents’ quality of care and life.
• Performs pre‑admission assessments and discharge planning.
• Participates in facility meetings and in‑service trainings.
• Participates in nurse on‑call rotation and promotes positive relations with residents and families.
• Collects and assesses psychosocial data and responses to interventions.
• Completes assigned portions of the MDS and maintains referral tracker.
• Coordinates admissions, discharge planning and care plan meetings.
• Assists with advanced directive decisions and grievance processes.
• Coordinates scheduling of allied health services.

The Admissions Coordinator makes assessment for, plans and advocates for the residents of the Community so they can reach their highest practical level of physical, mental and psychosocial well-being. The Admissions Coordinator will be responsible for pre-admission assessment of referrals as well as discharge planning for all the residents of the Community. If the selected candidate is an LPN, continued employment is contingent upon enrollment and active progress in an accredited Registered Nursing (RN) program. The employee agrees to begin the RN program within six (6) months of the employment start date and to complete the program within its standard duration.
