Job summary

Pivotal Health Care is seeking a dedicated MDS Coordinator - RN for Cedar Ridge Village in West Des Moines, IA. You will lead MDS 3.0 care planning, coordinate resident assessments, and advocate for resident-centered care within a skilled nursing unit.

The role emphasizes collaboration with nursing leadership, on-call rotation, and opportunities for RAC certification paid by the company. Prior MDS experience strongly preferred.

Qualifications

  • RN license current in the state of practice.
  • Experience with MDS 3.0 and RAC certification preferred; company may sponsor RAC training.
  • CPR certification required.
  • 1–2 years supervisory experience in a geriatric setting preferred.

Responsibilities

  • Timely completion of MDS 3.0 and submittals to CMS.
  • Oversees the Care Plan initiation and Care Plan updating process.
  • Assists in all phases of the delivery of nursing care for residents.
  • Initiate preventive and restorative nursing procedures to maximize function and independence.
  • Assess current residents to ensure proper level of care and documentation.
  • Promote resident-directed/centered care within the nursing department.
  • Provide orientation and training for staff.
  • Support nursing department continuing education and program development.
  • Be proficient with computers and health information technology as it relates to nursing.
  • Assist with survey processes and regulatory compliance.
  • Participate in on-call rotation with nurse leadership.

Schedule

This position is part of a strong nurse management team, utilizing a very high ratio of nurse managers to residents to oversee a small skilled nursing unit within a continuing care retirement community. The role is primarily weekdays, with a predictable on-call rotation, and flexibility to balance hours during the week.

Role

We are seeking a dedicated MDS Coordinator - RN (Registered Nurse) to join our team! The MDS Coordinator is responsible for completion of the MDS 3.0. The MDS Coordinator possesses strong assessment skills. Has the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, allied health professionals, and the community.


We are seeking someone who has MDS experience or is willing to attend MDS training and become RAC-certified, paid for by the company.


Responsibilities


  • Timely completion of MDS 3.0 and submittals to CMS.

  • Oversees the Care Plan initiation and Care Plan updating process.

  • Assists in all phases of the delivery of nursing care for residents in the nursing facility.

  • Initiate preventive and restorative nursing procedures for each resident so as to achieve the highest possible degree of function, self-care, and independence based on resident choice, where practicable.

  • Assist with assessing current residents to ensure proper level of care is provided and documented.

  • Understands, practices and communicates resident directed/centered care on a daily basis as it relates to the nursing department.

  • Assist with providing orientation/training for staff.

  • Assists and helps develop nursing department continuing education, in-services training and program development.

  • Proficient with computers and have the ability to learn health information technology as it relates to nursing.

  • Helps support and manage all nursing/ clinical aspects of state or federal government survey processes and regulations.

  • Participates in on-call rotation with other nurse leadership team members.


Qualifications

The MDS Coordinator must be a RN from an accredited college, be currently licensed in state of which applying without encumbrance, be responsible for maintaining professional licensure and CEU requirements.If the selected candidate is an LPN, continued employment is contingent upon enrollment and active progress in an accredited Registered Nursing (RN) program. The employee agrees to begin the RN program within six (6) months of the employment start date and to complete the program within its standard duration. Proof of enrollment and documentation confirming the program’s expected completion timeframe must be provided upon request.



  • It is preferred that the MDS person has 1- 2 years supervisory experience in a geriatric setting.

  • Current CPR certification required.

  • Has MDS experience or is willing to attend MDS training and become RAC-certified, paid for by the company.
