This position is part of a strong nurse management team, utilizing a very high ratio of nurse managers to residents to oversee a small skilled nursing unit within a continuing care retirement community. The role is primarily weekdays, with a predictable on-call rotation, and flexibility to balance hours during the week.

We are seeking a dedicated Admissions Coordinator to join our team! The Admissions Coordinator makes assessment for, plans and advocates for the residents of the Community so they can reach their highest practical level of physical, mental and psychosocial well-being. The Admissions Coordinator will be responsible for pre-admission assessment of referrals as well as discharge planning for all the residents of the Community. Has the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, allied health professionals and the at large community.
Must be a graduate from an accredited school, be currently licensed RN or LPN in state of which applying without encumbrance, and be responsible for maintaining professional licensure and CEU requirements. If the selected candidate is an LPN, continued employment is contingent upon enrollment and active progress in an accredited Registered Nursing (RN) program. The employee agrees to begin the RN program within six (6) months of the employment start date and to complete the program within its standard duration. Proof of enrollment and documentation confirming the program’s expected completion timeframe must be provided upon request.
Prefer 2 year experience working in a geriatric setting.
Must possess a valid drivers license
