Linden Woods Village in Gladstone, MO is seeking a dedicated Admissions Coordinator to assess and advocate for residents to help them reach their highest well-being. This role performs pre-admission assessments, discharge planning, and partners with families, physicians, and the care team to ensure resident-centered care and smooth admissions.

The coordinator actively participates in nurse on-call rotations, coordinates care plans, and ensures timely communication across departments.

Responsibilities

• Pre-assess residents as needed and participate in facility meetings.

• Collect psychosocial data and support care planning meetings.
• Maintain referral tracker and report monthly discharges to Ombudsman.
• Coordinate and assist with resourcing and scheduling of allied health services.

The Admissions Coordinator makes assessment for, plans and advocates for the residents of the Community so they can reach their highest practical level of physical, mental and psychosocial well-being. The Admissions Coordinator will be responsible for pre-admission assessment of referrals as well as discharge planning for all the residents of the Community. If the selected candidate is an LPN, continued employment is contingent upon enrollment and active progress in an accredited Registered Nursing (RN) program. The employee agrees to begin the RN program within six (6) months of the employment start date and to complete the program within its standard duration.
