Job Overview

The LPN Medication Care Manager is responsible for providing the highest quality care by administering medication and treatments safely and organized. Responsibilities include medication administration, documentation, conducting SHUs, and providing resident care, all in accordance with laws, regulations, and Sunrise policies, to promote the highest quality of life for our residents.

Responsibilities & Qualifications

Responsibilities:

• Support Sunrise’s Mission, Principles of Service, and Core Values, emphasizing the sacred value of human life.
• Administer medications, assist with treatments, and document accordingly, following the Six Rights of Medication Pass.
• Ensure confidentiality and proper documentation of all resident information.
• Report resident concerns and assist in medication checks and restocking.
• Maintain a clean and organized medication area.
• Assess residents for needs of 'as needed' medications and document appropriately.
• Participate in safety and risk management practices, including reporting incidents and maintaining safety protocols.
• Contribute to team success through active participation in meetings, training, and engagement initiatives.
• May serve as shift supervisor or supervise other medication care managers.

Qualifications:

• Licensed as an LPN, LVN, or equivalent in the relevant state/province, with current certifications including CPR and First Aid.
• At least 18 years old, with preferred experience working with seniors.
• Strong communication skills, good judgment, and the ability to handle multiple priorities.
