Job summary

Curana Health is seeking nurse practitioners or physician assistants to deliver high-quality primary care to seniors in skilled nursing and living communities. You will assess health, manage chronic conditions, and build lasting patient relationships in a role that offers autonomy and strong support from physicians and leaders.

You will work with care teams, leverage advanced technology and EMR tools, and travel between facilities as needed.

Qualifications

• Valid NP or PA license in state of operations and board certification.
• Masters degree in nursing or PA studies.
• Strong leadership, interpersonal and problem-solving skills.

Responsibilities

• Perform comprehensive health assessments and develop treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Collaborate with physicians, clinical staff, facility leadership, specialists, and families to coordinate care plans.
• Maintain accurate and timely documentation in the EMR and ensure regulatory compliance.
