Qualifications

• Active NP or PA license in the operation state with current board certification.
• Master's degree in Nursing or Physician Assistant Studies.
• Strong leadership and interpersonal skills; problem-solving ability.
• Excellent written and oral communication skills.
• Punctual, organized, and able to work well in a team.

Responsibilities

• Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build meaningful patient relationships through regular facility rounds and collaboration with families and care teams.
• Collaborate with physicians and care teams to coordinate comprehensive care plans.
• Maintain accurate and timely documentation within the EMR and ensure regulatory compliance.
