Formación

• Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field
• Masters Degree in Nursing or Physician Assistant Studies
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills
• Traits of punctuality, organizational skills, and ability to function within a team

Responsabilidades

• Deliver High-Quality Primary Care: Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build Meaningful Patient Relationships: Conduct regular facility rounds and develop strong relationships with residents, families, and care teams.
• Collaborate Across the Care Continuum: Partner with physicians, clinical staff, facility leadership, specialists, and family members to coordinate care plans.
• Maintain accurate and timely documentation within the EMR and ensure compliance with regulatory standards
