Job summary

Curana Health is seeking Nurse Practitioners or Physician Assistants to deliver proactive, relationship-based primary care to seniors in skilled nursing, assisted living, and independent living communities. Responsibilities

• Perform health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Conduct regular facility rounds and develop strong relationships with residents, families, and care teams.
• Monitor changes in health status and proactively intervene to prevent complications and unnecessary hospitalizations.
• Educate residents and caregivers on disease management, preventative care, and wellness strategies.
• Partner closely with physicians, clinical staff, facility leadership, specialists, and family members to coordinate comprehensive care plans.
• Serve as a trusted clinical resource within the facility and contribute to interdisciplinary care discussions.
• Support initiatives focused on improving patient outcomes, enhancing quality measures, and reducing avoidable hospital readmissions.
• Maintain accurate and timely documentation within the EMR and ensure compliance with standards.
