Formación

• Valid nurse practitioner or physician assistant license in state of operations
• Master's Degree in Nursing or Physician Assistant Studies
• Strong leadership, interpersonal, and problem-solving skills
• Excellent written and oral communication skills
• Punctuality, organizational skills, and ability to function within a team

Responsabilidades

• Deliver high-quality primary care in skilled nursing, assisted living, and independent living communities
• Develop individualized treatment plans and manage acute and chronic conditions
• Build meaningful patient relationships and conduct facility rounds
• Coordinate care across physicians, staff, and families
• Maintain accurate EMR documentation; ensure regulatory compliance
