Harbor Health in Kyle, TX is seeking a Nurse Practitioner or Physician Assistant to join our multi-specialty clinic. The ideal candidate provides high-quality primary care within a collaborative clinical team, delivers leadership in patient management, and supports our mission to transform healthcare in the payvider model.

You will conduct exams, manage chronic conditions, perform preventive care, and coordinate with specialists.

Qualifications

• Graduate of an accredited Physician Assistant Program and certified by NCCPA or Nurse Practitioner Program and certified by ANCC or AANP
• Current licensure to practice in Texas as NP or PA
• Licensed in Family Medicine and able to obtain privileging
• Current DEA and CPR certifications
• Valid Texas Driver's License
• Demonstrated proficiency with electronic health records and credentialing requirements

Responsibilities

• Interview members to obtain medical history and perform physical exams
• Order tests, prescribe meds, and recommend treatments
• Provide continuity of care for chronic conditions
• Make referrals with care team collaboration
• Perform physical exams for employment and other purposes
• Document and code all member contacts accurately in the EMR
• Contribute to clinical committees and quality reviews
