Job summary

Harbor Health in Plano, TX is seeking a team-oriented Nurse Practitioner or Physician Assistant to join our multi-specialty clinic at the Preston Park practice. As a clinical leader in our primary care setting, you will deliver effective, efficient care and collaborate with the clinical team to support a payvider model.

You will conduct comprehensive exams, manage chronic conditions, and participate in quality initiatives while adhering to credentialing requirements and maintaining high patient

Qualifications

• Graduate of an accredited Physician Assistant Program and certified by NCCPA or Nurse Practitioner Program and certified by ANCC or AANP
• Current licensure as a Nurse Practitioner or Physician Assistant in Texas
• Licensed and certified to practice Family Medicine
• Current DEA and CPR certification
• Valid Texas Driver's License
• Credentialing and privileging readiness

Responsibilities

• Interview members to obtain medical history, perform physical exams, order tests, prescribe medications, and recommend treatments
• Provide continuity in managed care for members with long-term conditions
• Make referrals for secondary and tertiary care with care team collaboration
• Perform medical procedures according to issued privileges
• Provide health maintenance visits and immunization guidance
• Conduct employment-related and other contract-specific physical examinations
• Conduct STD screening and family planning
• Document and code all member contacts in the EMR
• Function as an active member of the medical team
• Participate in and manage care team meetings
• Contribute to clinical committees, education, and quality review
