Harbor Health in Austin, TX seeks a Nurse Practitioner or Physician Assistant to join our multi-specialty clinic. You will provide patient-centered care within a collaborative payvider model and lead clinical decision-making in a primary care setting.

You will work with a diverse team, perform exams, manage long-term conditions, and ensure accurate EMR documentation while supporting organizational initiatives and quality care.

Qualifications

• Graduate of an accredited Physician Assistant Program and certified by NCCPA, or Nurse Practitioner Program with certification.
• Current licensure to practice in Texas as NP or PA, and Family Medicine certification.
• Current DEA and CPR certifications and willingness to meet credentialing requirements.
• Valid Texas Driver's License.
• Ability to interact with diverse populations and work as part of a clinic team.

Responsibilities

• Interview members to obtain medical history and perform physical exams.
• Order labs and other tests, prescribe medications, and recommend treatments.
• Provide continuity of care for patients with long-term conditions.
• Make referrals for secondary/tertiary care with the care team.
• Perform procedures per privileges and conduct health maintenance visits.
• Document contacts accurately in the EMR and participate in team meetings.
• Contribute to clinical committees and quality improvement initiatives.
• Maintain credentialing and privileging requirements.
