Job summary

Rutherfordton Area Post-Acute Care Practice under TeamHealth in Rutherfordton, North Carolina is seeking a quality-driven nurse practitioner or physician assistant to join our post-acute care team. This full-time role offers five days per week of rounding in skilled nursing facilities with strong support, training, autonomy and earning potential.

Qualifications

NP or PA license (North Carolina) and DEA registration required.
Experience in post-acute, acute, ED, or clinic settings preferred; new grads welcome to apply.
Strong clinical foundation and knowledge of healthcare regulatory standards.
Excellent communication and interpersonal skills.
Collaborative, team-oriented approach.

Responsibilities

Perform comprehensive patient assessments, including medical histories and physical exams.
Develop, implement, and adjust individualized care plans based on patient needs.
Monitor progress and coordinate care to optimize outcomes.
Collaborate with physicians, nursing staff, and other healthcare professionals in a team approach.
Educate patients and families on health management and preventive strategies.
Maintain accurate, timely medical records in compliance with regulations.
