Essentia Health in Duluth, MN seeks a Nurse Practitioner or Physician Assistant to provide comprehensive health care with a strong team-based approach across clinic, inpatient, and dialysis settings.

The role emphasizes health promotion, disease prevention, and complex chronic disease management, with travel between sites and dialysis rounding dominating the schedule. Licensure in the appropriate state and board certification are required.

Qualifications

• Current licensure as a registered nurse in appropriate state.
• Current licensure as a Nurse Practitioner in appropriate state.
• Current licensure as a physician assistant in appropriate state.
• Current CPR certification.
• National certification in applicable area.
• Successful completion of Essentia Health credentialing process prior to practice.

Responsibilities

• Provide health care to individuals and families with emphasis on health promotion and disease prevention.
• Assist Physicians with non-encounter work.
• Work in nephrology across clinic, inpatient, and dialysis units for all phases of care.
• Travel between Duluth area and nearby locations as needed; 90% dialysis rounding.
• Collaborate with APPs and MDs to deliver team-based care.

Utilizes advanced health assessment, knowledge, and decision making skills, works independently and in collaboration with physicians and other health care professionals. Provides health care to individuals and families, emphasizing health promotion and disease prevention.This position will assist our patients by utilizing a strong team-based approach to care, with a high integration of APP and MDs for the clinic, and inpatient and dialysis units for all 3 phases of nephrology care.· Desired skill set: Experience in complex chronic disease management· Typical Shift: up to 0.4 FTE per pay period· 5% clinic work· 5% hospital work· 90% dialysis rounding· Ideal candidate does not need to live in the Duluth area; candidate could live in areas near Virginia, MN or Grand Rapids, MN as there is travel involved with this position· Schedule: Variable start and end times due to travel· Estimated Patients Per Day: 12 (office), 12-15 (dialysis).
