Job summary

Impact Health PC is seeking an experienced Nurse Practitioner to provide Medical/Primary Care within skilled nursing facilities in Longmeadow, MA. You will assess, treat, and manage acute and chronic conditions, and collaborate with physicians and care teams to optimize patient outcomes.

You will work with autonomy within your scope, while receiving strong clinical support and access to resources to succeed. Flexible scheduling and a patient-centered approach define the role.

Qualifications

• Active NP license in Massachusetts with current credentials.
• Minimum 2 years of post-certification clinical experience preferred.
• Experience in skilled nursing facilities or primary care settings is preferred.

Responsibilities

• Provide comprehensive Medical/Primary Care to residents in skilled nursing facilities.
• Assess, diagnose, and manage acute and chronic conditions with evidence-based plans.
• Conduct routine patient evaluations and ongoing follow-up care.
• Collaborate with physicians, staff, specialists, and care teams.
• Educate patients and families on conditions and treatment options.
• Maintain regulatory compliance and accurate clinical documentation.
